2026 Strategic Grant Funding Application-New Funding Partner

Introduction

Dear Community Partner:

Augusta Health will offer funding opportunities for programs focused on the priority areas of need identified in the most recent Community Health Needs Assessment (CHNA). August Health's Board of Directors and its Community Partnership Committee oversees the funding process and approves funding recommendations as part of its overall focus on community health improvement.

The latest CHNA is available on Augusta Health's website at https://www.augustahealth.com/service/community-outreach/community-health-needs-

assessment/. Programs with intended outcomes in the following priority areas will be considered:

Access to Health Care

Mental Health and Substance Use

Chronic Disease Prevention

Housing

Applications will also be considered for Community Building programs, which address root causes of social determinants of health in our community (ex. workforce development, transportation, education, etc.)

In addition to serving the residents of Staunton, Augusta County, Waynesboro and surrounding communities, organizations eligible for Strategic Funding grant funding must have:

Designation as a 501(c)3 nonprofit, tax-exempt organization, or serve as a public agency

Demonstrated core competency in a priority area of focus listed above.

A program that is ready for implementation in January 2027, and includes an evaluation

method for measurable outcomes to demonstrate the effectiveness of an evidence-

based program. Funds awarded must be spent in Calendar Year 2027.

A track record of consistent demonstrated outcomes following program implementation,

as well as grant report compliance.

Organizations are asked to provide photos of the funded program to be used on Augusta Health's social media platforms.

High priority will be given to collaborative programs involving multiple organizations and those intended to make a significant impact on the area of identified need.

Grant applications for new funding partners can range from $2,500-$5,000. Future Strategic Funding grants can range from $2,500-$25,000. Applications for funding are available through

Kaleidoscope and must be submitted by 11:59 pm on September 15, 2026.

Funding progress reports are required in June and December of 2027, to be submitted

electronically through our grant software online.
