2026 Community Contributions Grant Funding Application

Introduction

Community Contribution Funding supports programs and initiatives in Augusta Health’s service area that create sustainable and positive change in the health of our community.  The maximum amount available is $2,500.  Applications are considered monthly throughout the year.

Qualifications

The focus of Community Contribution funding must be to improve the health of our

community in one of the following areas of identified need:

  • Access to Health Services

  • Cancer

  • Diabetes

  • Heart Disease & Stroke

  • Infant Health & Family Planning

  • Injury & Violence

  • Mental Health

  • Nutrition, Physical Activity & Weight

  • Oral Health

  • Potentially Disabling Conditions

  • Respiratory Diseases

  • Social Determinants: Housing

  • Substance Abuse

  • Tobacco Use

Selection Process
