Road Type: State, County or Municipal
Pickup Type: Routine Maintenance or Hot Spot
Miles Cleaned: Total number of miles picked up.
Bags Collected: Total number of bags collected.
Pounds Collected: Total weight collected.
Remarks: Additional comments/items of interest.
Include any supportive documentation. Supportive documentation includes (but not limited to):
Before and after photos.
Copies of all related newspaper articles.
Letters of commendation and other related documents.
